The Phoenix at Braselton is seeking an Assistant Program and Outreach Director (Activities Assistant) to join their team!

Shift Details

• Activities: Monday to Friday; some weekends required

The Assistant Program and Outreach Director reports directly to the Program and Outreach Director.

SUMMARY

The Assistant Program and Outreach Director is responsible for assisting in the day-to-day delivery of resident-centered activities that promote and integrate the Phoenix Senior Living philosophy to incorporate wellness in all aspects of the seniors’ lives we serve. This includes ensuring all dimensions—emotional, sense of purpose, physical, social, intellectual, and spiritual—are incorporated into the services provided and embraced.

RESPONSIBILITIES

• Refers to the Resident Profile and addendum for every new resident.
• Assist with facilitating a variety of appropriate and personalized activities that incorporate all wellness dimensions, available throughout the day and evening, with resident care associate participation.
• Understand the recognition of resident changes in condition and take appropriate action.
• Assist residents to and from activity rooms when necessary.
• In memory care, assist residents to and from Life Skills and other normalizing, life-enriching activities and routines, and assist with engagement.
• Review, read, notate, and initialize communication logs to document and learn pertinent information and any resident’s physical and behavioral changes.
• Maintain activity areas in an orderly manner.
• Assist with delegating daily care of any animals or plants within the activities program and services, as well as maintaining related records.
• Assist in maintaining an inventory of activity and programming supplies, including games, programs, and crafts.

Requirements

Education/Experience/Licensure/Certification

• High School Diploma required; job-related experience preferred in a senior living community.
• Ability to lead activities and motivate volunteers and fellow associates for involvement in social events and scheduled activities.
• Knowledge and experience in the Assisted Living industry and Dementia care are preferred.

Skills and Abilities

• Demonstrated ability to schedule, orient, and train staff.
• Understanding of infection control procedures.
• Ability to plan and organize effectively.
• Ability to multi-task and manage stress.
• Proficient in MS Office applications.
• Understanding and assistance with developing and implementing Personalized Service Plans.
• Knowledge of advanced directives and end-of-life care.
• Embraces the assisted living philosophy.
• Knowledge of state regulations and proficiency with compliance tools.
• Effective time management skills and adherence to deadlines.
• Availability to work weekends, evenings, and flexible hours to meet peak service times.
• Valid driver’s license (such as CDL or chauffeur license) with an acceptable driving record in accordance with Federal Department of Transportation standards.
• Exceptional grammar and documentation skills.
• Basic understanding of risk management concepts.

Physical Requirements

• In an 8-hour workday, may stand/walk for 4–6 hours, with 2–4 hours at a time.
• Sit for 2–4 hours in an 8-hour workday, with 0–2 hours at a time.
• Drive for 1–3 hours per week, 1–2 times.
• Support or assist with lifting or carrying up to 50 lbs regularly and occasionally up to 150 lbs.
• Lift/carry up to 40 lbs frequently and occasionally up to 70 lbs, with lifts occurring at heights of 3–4 feet and over distances of 30 yards.
• Use hands for repetitive simple grasping, pushing, pulling, and fine manipulation.
• Ability to bend, squat, kneel, and climb frequently, and reach occasionally up to 3 feet.
